While Blue Sky is happy to bill the listed insurance companies for all services rendered, your coverage is an agreement between you and the insurance company. Fees not covered by insurance for any reason are your responsibility. I will work with you and the insurance company to resolve any of your denied claims; however, all unpaid portions are your responsibility and payment is expected within 30 days of billing.
EVALUATIONS
SPEECH/LANGUAGE ASSESSMENT
Assessment measures and evaluation report for speech and basic language skills.
LANGUAGE ASSESSMENT IN THE ABSENCE OF SPEECH CONCERNS
Includes different assessment measures relating to language, communication and auditory processing abilities.
LANGUAGE AND LEARNING ASSESSMENT
Includes parent interview, review of all pertinent history, comprehensive testing measures and evaluation report.
THERAPY
It is expected that families will respect the therapy process and show up for scheduled appointments. There is a fee for appointments cancelled with less than 24 hours notice and for failure to show up (without notice). This fee is not covered by insurance.
